Your privacy is important to me
One of the most important elements of confidentiality is that it helps to build and develop trust. It potentially allows for the free flow of information between the client and Counselor and acknowledges that a client's personal life belongs to them.
All forms will need to be filled out in my office prior to your 1st session due to my very strong concern about maintaining my client's privacy/confidentiality.
Even with the most up-to-date computer security software there is still a danger of someone hacking anyone's system and compromising client's personal information.
An example of some additional forms that will be required is below:
- Adult Questionnaire Form
- Authorization Form
- Child Questionnaire Form
- Consesnt for Treatment Form
- Policies and Procedures Form
